This position is eligible for American Rescue Plan Act (ARPA) funds which includes a monthly stipend of up to $750.00 which is based upon the hours worked in the previous month. These funds are guaranteed through June 30, 2023.

About Your Team

Child & Family’s Aging Well Program has been providing vital support services to seniors in Newport County since the 1960s and to seniors in Providence since 2000. We support clients in obtaining resources to address their needs from housing to mental health services. Your team strives to enhance seniors’ quality of life by focusing on preserving their dignity and respecting their independence.

What You’ll Be Doing

As a case manager for the Adult Protective Services program under RI’s Office of Healthy Aging (OHA) in Child & Family’s Aging Well program you will be assisting one of the most vulnerable populations in our community navigate incredibly delicate situations dealing with elder abuse and self-neglect. You will assess their needs and determine the best next steps.

 

Your Main Priorities

While you will likely have additional responsibilities arise on the job, primarily you’ll:

  • Manage cases assigned by intake coordinator
  • Conduct initial in-home assessment to determine care needs
  • Create care plan and recommend services to address individual needs
  • Follow-up with clients as needed per the guidelines of OHA after initial plan implemented to reassess needs
  • Collaborate with family/caregivers, service providers, and local authorities, as necessary
  • Complete all paperwork promptly and maintain accurate client records
  • Advocate for services to meet elder’s needs

  • Associates degree (required)
  • 1+ year in case management role (preferred)
  • Experience with APS a plus
  • Bilingual (required)
  • Valid driver’s license & reliable vehicle with valid registration & insurance
