Case Manager
This position is eligible for American Rescue Plan Act (ARPA) funds which includes a one-time $750.00 sign-on bonus payable after the completion of 90 days of employment as well as a monthly stipend of up to $694.50 which is based upon the hours worked in the previous month. These funds are guaranteed through June 30, 2023.

Job Summary
The Caseworker is to provide social service home-based services to individuals and families who are experiencing stress in coping with physical, emotional or social difficulties. Identify the needs and provide support as needed as well as matching the individual or family with appropriate services to promote and maintain families living together and work as a team with the Clinical Caseworkers, when clinically indicated. Respond to families in crisis throughout the area stipulated in the DCYF contract. Due to the nature of the workflow and level of responsibility this individual maintains a flexible schedule, which would include nights and weekends, alternating on-call duties are also assigned.

Responsibilities and Duties
Provide support to individuals and families who are in crisis and/or are unable to provide proper care and nurturance to their children.
Interview clients individually, in families, or in groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs; Provide initial consultation to families to review the service planning process
Relate observations and impressions of clients' situations as determined necessary to coordinate services. At other times it is expected that the Caseworker will exercise her/his own judgment to implement the social service plan most effectively.
Counsel individuals, groups, families, or communities regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care.
Consult with parents, teachers, and other school personnel to determine causes of problems such as truancy and misbehavior, and to implement solutions.
Implement a specific service plan developed by DCYF, the family, and the EFSS Caseworker which is designed to improve behaviors and communication with and among family members; review service plans in consultation with clients, and perform follow-ups assessing the quantity and quality of services provided.
Utilize home-based strength model to provide a wide range of assistance and resources to families in the process of reunification or to avoid separation.
Schedule and coordinate case conferences and meetings with service providers.
Provide training and support to parents and caregivers, including:
Assist caregivers in development of formal and informal support systems through increasing awareness of available resources and assist in linking families with those supports
Encourage caregivers to make and keep appointments by identifying and eliminating barriers to transportation and/or by accompanying the parent to and from appointments
Aid families in establishing time management and stress reduction techniques
Assist parents in identifying their child's developmental needs
Improve parents’ child management skills through modeling, education, support and encouraging proper parental coping skills
Provide support through family mediation and teaching skills to help families resolve conflicts
Help conduct parenting classes appropriate for both families with young and adolescent children
Broaden parents’ knowledge of developmental stages appropriate for their children
Qualifications and Skills
- Bachelor's Degree in Social Work or related field - will accept equivalent experience in child welfare services.
- Knowledge of child and adolescent development, family dynamics, substance abuse, home management, parenting skills and crisis intervention.
- Ability to relate to clients in a supportive, empathic manner, appreciation and respect for the difference and needs of others.
- Thorough understanding of the stages of child development.
- Prior experience in child welfare, parent education, home-based services, crisis intervention, work as a Family Support Worker, work with multi-problem families.
- Requires a valid driver’s license; CANTS, BCI and fingerprinting background check.
