Case Manager
This position is eligible for American Rescue Plan Act (ARPA) funds which includes a one-time $750.00 sign-on bonus for newly hired staff hired prior to April 1, 2023, payable after the completion of 90 days of employment as well as a monthly stipend of up to $750.00 which is based upon the hours worked in the previous month. These funds are guaranteed through June 30, 2023.

Job Summary
Overall clinical responsibility for assessing older persons, developing plans of care, arranging for and monitoring services. Assessment goals are to identify needs, make referrals to appropriate service systems, and follow-up. Position will help improve the quality of life for those elders and promote independent living by providing counseling, advocacy, education and support systems to the elderly living in the community.

Responsibilities and Duties
- Conduct comprehensive in-home assessment of older person’s needs, using standardized tool provided and within established response time
- Provide monthly summaries to DEA
- Assist in telephone screening of elders referred to the program
- Maintain a working knowledge of the payer requirements
- Develop plan of care from assessment and UCAT or CNFA to address needs and problems identified and to incorporate services needed to enhance current support system
- Arrange for services through direct authorization from State, specifying amount, scope and duration of specific services and/or coordination and tracking of service referrals
- Provide case management services to Protective referrals from Office of Healthy Aging
- Monitor changes in elder, circumstances and/or provision of services, through telephone and in-person contact with elder, information and formal support system. Make changes in plan of care as indicated
- Advocate for services to meet elder’s needs, on an individual, community-wide, and/or system-wide basis
- Maintain positive working relationships with staff of other service agencies. As requested, represent program at various aging network meetings
- Communicates with other service providers and payer sources regarding the patient's needs, plan and response to care
- Assesses the clients within the caseload to identify needs, issues, resources and targeted outcomes
- Maintains appropriate documentation of patient care and progress within the plan

- Associates Degree
- Spanish speaking preferred
